depends how he treated you while u at work, how long u were there and if u r willing to keep relation with the company.
In general write a letter with few good thing about company, boss and co-worker, say how appreciate u were, but u need to leave for some advancement etc.

I sent mine an e-mail which was easy but if you can't do that just tell them you need to talk to them when they have the chance and just say I just wanted to give you my two weeks bc whatever. My reason was bc Im moving so she was understanding. Good luck!
