Doctors often took eggs, chickens, other food or materials as payment because people simply did not have the money to give to the doctor. so, as payment for services, many people gave the doctors things they could use or things they needed.

I'm going out on a limb here, but I'm guessing that your Gram is in her 70's-90's or higher, so here is a possible answer:

Back in the early 1900's and even earlier for taht matter, money was scarce, so people bartered for they're food and livelyhood. for example: If there was a farmer who had a crop of grain, he would trade it to other farmers for something that they had, perhaps eggs, or meat, or corn.

Maybe you should check the old TV show Little House on the Prarie.
