If you are talking about Post-graduate education then it means that it's education for people who already graduated from a 4 year institution with a B.S. or B.A. degree and who apply for graduate school on a program to get an advanced degree. A grad school could be like med school, law school, or just a program from which you'll obtain a masters degree or a higher degree like M.D. or PhD which is the highest degree on many careers.

I think you are referring to "post-graduate" as an adjective. It means that portion of a university which is above the bachelor's degree level. You graduate when you get your BA or BS, then take a post-graduate degree when you go for a master's or a doctorate.

As you can see, Post Graduation is made of two words; Post and Graduation.
Post stands for posterior and it means 'after' and 'graduate' is an academic degree from a school or a college.
So it means any academic degree pursued after completion of this academic degree called graduation.

In the U.S., we do not use this phrase. We refer to "graduate education" or "grad school" -- which means the master's degree and/or the Ph.D. We call the bachelor's degree (baccalaureate) an "undergraduate" degree. In the U.S. system, we have 12 years of primary and secondary education -- this concludes with a high school diploma. After that, the 4-year bachelor's degree.

A Post Grad is someone who is still taking classes (degree seeking or not) but who has already attained a Bachelor's degree.
