it depends on if the treadmill is moving or not, and how long it takes you to run the length of it.
if a treadmill is moving at 10 mph (very fast) and it takes you an hour to run the length of it (1 mile) then you've run 11 miles.
if it isn't moving at all, then yes, you've run a mile.

If you run the actual length of the treadmill, as in the back to the front, then yes you have run one mile. If you have just been running in place at the pace the treadmill is turning then no you have not. You have not changed position, but you expended the same amount of energy as you would have if you had actually run a mile.

Yes, of course you will run 1 mile. If it is on however, you must think of it a bit differently. If a treadmill is in motion, you technically won't move an inch if you're keeping up with the motion but 1 mile of this treadmill will have gone by. Thus, you've effectively run a mile yourself.

If the treadmill is turned off, yes. You would need to know the speed of the belt. Distance = rate x time. You need to know two to arrive at the third.
