I WAS a cook and a chef for about 15 years. I got tipped a few times and it always made me feel great, no matter what the amount was. Just that somebody thought to do it.

No!

Guidelines for Tipping

People in the service industry can be among the hardest working people you will ever meet. Most of them are paid a paltry hourly wage, and they depend on tips to make a living. There are standard guidelines for tipping.
In restaurants, tip based on the service, not the food. You are not tipping the chef, you are tipping the waiter/waitress.
For good service, you should leave 15% of the total bill. If you are using a coupon or other discount, the tip should be based on the full bill, not the discounted amount.
If the service was outstanding, 20% is a great way to show it. Alternately, if the service was below par 10% or lower sends the same message.
Bellman should normally be tipped $3 per bag when delivering bags to your room. It is customary to tip both upon arrival and departure.

No you do not have to tip the chef or cooks, but if you loved the food it is a nice thing to do. And to all of you who think that the cooks are getting paid well, think again. Most cooks make less than $10/hr, unless they have a lot of experience and are good at what they do.

Yes, at the sushi bar, it's good courtesy, especially if he makes a great special roll, and you go there all the time.

No but I have gone to the door of the kitchen to say thanks and congrats on his/her amazing food. But that was only once with a friend of my dad's who came out himself to get pride and smoked a ciggarette.
