Actually, people can become dependant (i.e. addicted) to counseling. This can become serious if the person has to rely on a counselor in order to make decisions and function as a normal adult.
As to how to treat them; I don't really know. I suggest you go to a psychologist and ask them this question. ;-)

Yes, someone could become dependant on counsellors. They would require the counsellor to intervene in decision making in order to function as a normal adult.
The person could try switching counsellors or the counsellor could slowly detach from them.
This is a rare case that happens in counselling because counsellors are trained to watch for this.

Yes, someone can become dependent upon their couselor and not take any responsibility to improve life on their own. It is a crutch just like drugs or anything else. It would be up to the counselor to slowly detach the person from this dependent behavior. Counselors are trained to watch for this in their clients.

It would be more of a crush,habit,letting out stress..... You get the point. You can only be addicted to a substance, not a person.

Yes, just as there are hypochondriacs. Can be many reasons, they love attention, no one else listens to them and mainly, I think, because they want someone else to decide issues for them, so they don't have to take responsibility for their own behavior.
Treatment..a tough love shrink!

Nope- I know it's a joke, but actually the counselor will "terminate" when you're cured. It just may take a long time.

Join them in a room list their problems and just stare at them for hours saying nothing, rinse and repeat as necessary.
