if you have been arrested by the police even a for a minor offence you need a visa, that's what the embassy told me. ( i once drove with no insurance on my car, stupid i know). If you are not sure go to your local police website and download a form. send it of to Scotland yard and they will reply will details of any outstanding convictions you have. immigration in America use this as well. if all clear just fill the waiver out on the plane. if not contact the embassy for a appointment.

Not if you're from the UK or many European countries, amongst others. You have to fill in a couple of Visa Waiver forms on the plane (make sure you do it carefully though!) If you're British, you don't need to get a visa. You qualify for the Visa Waiver Program. If not, a list of countries in the program is on this site: http://www.travel.state.gov/visa/temp/without/without_1990.html#2

No, we operate a Visa waiver programme (unless you have a criminal record or have been arrested).
You need a machine readable passport though.
