This is from the repentance recited in churches such as the Catholic and Anglican churches. It can be found in the prayer books of the church. I have the Anglican ASB 1980 and it seems to be a more modern version in that, so I believe you need to get a copy of the Book of Common Prayer. Try Amazon or Ebay, or your local church, which should have several.
