I know it's really cheesy and naive, but the actors/actresses make it look very real on TV and in movies. So i was wondering if they are doing the actual thing?

If not, are they wearing any underwears?
Sorry to be so bold and aks like this. Curiousity is a powerful thing.
I was watching "the notebook" and "Dracula 2000" and the people on screen looked pretty engaged and aroused, so yeah.........

The depiction of sexuality in mainstream cinema was at one time restricted by federal law and self-imposed industry standards. Beginning in the 1960s, however, mainstream cinema began pushing boundaries in terms of what is allowed on screen. Although the vast majority of sexual situations depicted in mainstream cinema is simulated, on rare occasions, filmmakers have produced motion pictures in which actors were allowed (or instructed) to engage in some level of genuine sexual activity, up to and including sexual intercourse. The difference between these films and pornography is that, while such scenes might be considered pornographic, the main intent of these films is usually not pornographic. Despite this, the release of such films (for example, Vincent Gallo's The Brown Bunny and Michael Winterbottom's 9 Songs) have often been accompanied by controversy; many of these films have only been released in their uncensored form on home video and DVD.

Some do. They even go all the way and ejaculate inside of the woman's vaginas. One actress got pregnant from a movie sex scene.

If the actor and actress are really lousy in other scenes and "spark" in the sex scenes the chances are good that they are actually having sex. The director will do anything to get a good performance from the actors and actresses.

Usually not. It is VERY difficult to do "on screen" intimate scenes. They are not at all like real sex, kissing, etc. You have to take into accoung the lighting and all that. Very uncomfortable.
