This particular Dr may not be listed as in his own practice but may be listed as with a group and without that groups name you are pretty much sunk.....unless.....
Contact your local medical society as he may be listed there.
Check with your local hospital in the administration dept. If that particular doctor has practicing privileges there it will be in his name and not that of the group in which he works with.......Good Luck
