A - You need to withdraw from a women PRIOR to losing your erection. This keeps the condom on and fitting properly.
B - You need to hold the base of the condom while withdrawing because it can slip off and remain in the vagina if you don't.
C - If for some reason the condom doesn't fit properly (or if it breaks) during intercourse, further thrusting will cause the condom to go to the back of the vagina.

Three main causes. Not putting it on right (not unrolling it all the way), wrong size (guy buys the jumbo because he thinks he is), or he did not pull out as soon as he finishes

wearing a larger condom then you need, Buy extra small condoms it might fit you better

not enough (feminine) lubricant. Happened to me my first time 25years ago after my first 'real' boyfriend's prom. It hurt like hell. My mother though was actually proud that I had safe sex.....go figure.......
