Kurt Cobain left a drug rehab center in Marina Del Rey California on April 1, 1994 and was later reported missing. As you probably know, he was found dead just seven days later.

Kurt died from suicide. He shot himself with a sawed off shot gun. He was bi polar and usually bi polar people if not treated properly kill themselves because their highs are so great they are flying they their lows are so massive that all feels doomed.
