a) perhaps your device cannot be supported by your windows operating system. For example: my Camera is not being reconized via usb plug and play.
Solution: your device should be installed manually with its installation cd. ( ehat ofcourse you can get it from itsbrand's website)

b) your usb ports are messed up by windows
solution: the best way is to reinstall your windows.

Could be a lot of different reasons. First thing you should do is open Device Manager, expand the USB groups, and see if it indicates an error with your USB hosts.

Start -> Control Panel -> System -> Hardware -> Device Manager -> Universal Serial Bus Controllers (expand it)

A bad device will have an exclamation point next to it. Right click on it and select "Properties".
