My wife and I are sensitive to smoke as well. The key is ventilation and HIGH CEILINGS. Many of the casinos along Fremont Street, for example, are older with low ceilings and not as good ventilation. We are slots players. Along the Strip, we've found that the newer Venetian, Paris, Caesars, Luxor, Bellagio, Aladdin, and Rio are OK most of the time. I'm guessing MGM Grand and New York New York, along with some of the other majors, also would be fine. IF a smoker sits down beside you (or worse -- two sit down one on either side) -- you'll probably just have to move. I believe some do have non-smoking sections (ask an attendant or security guard).
Here is another perspective from the Vegas.com web site: Q: What about smoking? Can I light up while I gamble?
A: Vegas is a smoker-friendly town, and every casino allows smoking. If you are looking for a smoke-free place to play, the Palace Station, Santa Fe Station, Texas Station, Bellagio and Golden Nugget have smoke-free poker rooms, and non-smoking tables are available at the Mirage.
And another answer from abouttravel.com\las vegas:
All right, all right, enough with the fantasy world and into reality. There are ways to enjoy gambling in Las Vegas and here are a few tips to stay clear of the smoke:
Gamble at the newer hotels. They have improved ventilation and air-filtration systems, which older hotels downtown do not have. It really makes a difference. Unless you are sitting right next to a smoker, it is hard to tell where the smoke is.
Avoid the crowds. Not too long ago, it was a ghost town in Vegas in the summer. Now, it is just as crowded as going to any other popular destination around the holidays or summertime. Less people means less smoke, so if possible schedule your trip around these times.
Gamble in casinos that offer designated smoke-free areas. Here is a list of a few:
• Bellagio designates some gaming areas, including the poker room as non-smoking. They evaluate each pit in the casino and make adjustments day-to-day for nonsmokers.
• Harrah’s has a 1,000 square foot non-smoking gaming section, which includes reel slots, video poker machines, blackjack and roulette tables.
• MGM Grand offers nonsmoking gaming tables and a designated nonsmoking slots section in the main casino.
• The Mirage has fresh air pumped into the poker room and contaminated air pulled out of the building. Not only does this help to keep the air clean, but rumor has it that oxygen is pumped in to keep gamblers awake so they will not leave the tables.
Virtually all showrooms are now smoke-free. Most hotel-casino convention centers are non-smoking. Indoor pools, family arcades and health spas are also generally smoke-free. Restaurants designate an average of 70% to the nonsmokers. The Venetian, Harrah’s, MGM Grand, and the Rio All Suite Hotel have 80% or more of their rooms designated as non-smoking rooms and suites.
As a non-smoker myself, I remember the days of the smoke filled casinos and coming out smelling like I had become a cigarette. Those days are gone, maybe not downtown, but it is not the same Vegas as it once was. The food, gambling, shopping, spas and beautiful hotels can be enjoyed by all, even those most sensitive to smoke

Many of the casinos have non-smoking areas and tables. One of the best casinos to gamble in and not be killed by the smoke is Bellagios. That's beacuse its a very spacious place and they also have good ventilation

nope, but a very few do have non-smoking sections. This is a town of every conceivable vice. Expecting smokers not to smoke while they gamble, drink, and anything else you can imagine is like giving a baby a bottle without any milk, it doesn't work..

I smoked for 40 years (quit 12 years ago) and I avoid Las Vegas just for this reason, even before I quit it bothered me!. I know it doesn't matter to the casino owners, but I refuse to be subjected to the stench of the smokers. Smoke free rooms and ventilation do not help. I have been in smoke free rooms where people decide to smoke irregardless and they do not enforce the policy., And the hotel rooms? Once they are smoked in, you can't get the smell out. I speak from experience. I truly believe a non-smoking casino/hotel would thrive there. Until they build one I won't be back!
