I would have to say the Normandy Invasion scene in Saving Private Ryan. That first 25 minutes is so intense. The guy walking around in the middle of a hail storm of bullets looking for his own arm, or the guy trying to hold his guts in while crying for his mother, those parts give me chills just thinking about them. That scene was so intense, there were grandmothers crying in the theater when I went to see it, it was that real.
The paradise in "What Dreams May Come" is the most unreal thing I have ever seen in a movie. It looked exactly like a painting, it was so trippy.

Pulp fiction, when Uma was OD'ing and they had to stab her in the chest with the adreniline.
Also, American werewolf in London- when he first changes into the werewolf.
Then there is 'Audition' Its a japanese film, and the entire thing creeps your out right until the end where this really pretty asian girl sticks needles in this guy and saws his foot of with a bit thing of wire while taking this really sadistic schoolgirl like pleasure in doing it.

I'll never forget it, it was The Thing with Kurt Russell. I saw it at a drive-in. When he put the hot wire in the dish of blood and the creature screamed, I banged my head against the window of our pick up truck, and my husband laughed his head off.
