Absolutely it is!
More so for psychological services. But a doctor would have an unfair power over the other person, know their weaknesses and be able to influence them in any course of treatment instead of allowing the patient to make an informed decision.

Depends. Doctor like just a basic doctor for checkups and whatnot, it's ok. If you're a surgeon... Not quite. Emotionally attached to patients with their life at stake = bad. Also could cloud your judgement.

I would say not unethical, but perhaps unwise if he does not take her seriously. I know of a woman who married her chiropractor, but I'm not sure that's the same thing. The big risk is that he will lose his objectivity in dealing with her medical problems.
