The experimenter -- may not have checked before measuring.
Or could be the mistake of the apparatus. While calibrating, wrong ethalon may have been used.

This question is not clear.

Assuming the experimenter was given an instrument that was not calibrated to calibrate, the error could be due to parallax.

I think it's b). "Manufacturing Defect" to be more precise.
