Just draft a letter, your name, addy, ifo in upper right corner. Begin letter w/ "To whom it may concern @ __________",
and simply say " Please use this letter as notice of my intention to vacate the premises of ________(address), on_____ (date). I will be moving at that time.
Sign it "thank you, "

You are not obligated to give 30 days notice when your lease is up. It's your landlords responsibility to provide you with the option to renew your lease or decline before your current lease expires. A lease is a written contract, when you fulfill that contract, you are under no further obligation. If your landlord is dragging his feet on providing you with a renewal to decline, thats his problem. It's not like you are abandoning the lease early.

You should just be able to go into your leasing office in the apartment complex and give them a verbal 30 days notice. Or you can write one and give it to the manager of the apartment complex in the leasing office. Just, write To Whom It May Concern: I _________, hereby give my 30 days notice. I will be moving out of __________________ (address of apartment) on the ___ day of _____ (month) ___ year. And, then sign it and give it to the leasing office manager.

Thats all there is to it . Just write this is my 30 notice before I move. Sign it date it make a copy and give one to your landlord
