Chevrolet Corvette would have the longest history, and Toyota Corolla would have the most model changes with the same name - that's what I think.
The VW beetle would also be a consideration, as the first production was in WW2, and the last ended about in 2002 in Mexico - and this was actually the same design!

Continuously, I'm pretty sure I heard that it was the Chevy Suburban. I believe they started making them back in the late 30s. The olds 88 had been around since 1949 too until they quit making them, so that was another long runner.
