There are many reasons as the to the responsibility of the teen aged, love angst lovers. Could it have been the priest who suggested the potion to Juliet and couldn't make it in time to tell Romeo ? Could it have been Romeo who was too emotionally overworked thinking he couldn't live without Juliet ? Was it their parents who forced them into this situation due to the family feud ?

In all actuality they are both responsible for their own deaths, as they were both suicides. But the literary answer would be fate. It is even hinted at early in the play when Juliet is talking to the Nurse, she says... "My grave is like to be my wedding bed".
