Forget the cleavage! When I'm interviewing for a position, I look for professional appearance and attitude and sexy does NOT impress! (A hiring manager who responds to that has ulterior motives; do you really want to work in that kind of environment?)
Dress conservatively, one step above the position you're interviewing for. Leave the excess jewelry at home. VERY light on the perfume, if at all. Conservative hairstyle. Absolutely squeaky clean.
Be straightforward and completely honest in your responses. Be engaging without being syrupy-sweet.
Have questions ready for the interviewer as well. Since you're being called back, as for more details on the new position.
Appear interested in the position.
Be on time! 5 minutes early is perfect; way too early is better than late, but isn't very good.
There are no canned phrases that will make you stand out from the crowd. We've heard all the goofy stuff already and it will only hurt your chances.
The fact that that they're calling you back for another position is EXCELLENT. You've already passed muster, really, and unless you truly fall on your sword you should get an offer this time around.

Meet on time, dress accurately; not too overdressed and never underdressed. Calm colors. Nice shoes, hands: short nails, makeup and hair: not hanging loose. Since its a surgery: as little jewlery as possible! = Hygiene. Think it over; why would you hire you? Why would they hire you? What are your assets? Be honest! Dishonesty allways shows - at one point. Do not ask questions you obviously must know the answer to - like what the advertisement told you. Ask relevant questions if you have some. Have eyecontact with all persons interviewing you and especially the one asking the questions.
At a doctor's surgery some important assets would be SERVICE; how do you meet people? CARING - how do you meet people? KNOWLEDGE - how do you meet people? That should make a nice impression.
Remember - they called you again - you should feel somewhat confident. Good luck!

Besides getting the first impression down, make sure you go in prepared. Research a bunch of commonly asked interview questions and prepare answers for them and practice giving the answers to a mirror so you can see your body language. Make sure you are confident in everything you say and how you present yourself. Finally, but most importantly, be organized. All responses that require sharing an experience should follow a STAR format.
S- Situation
T- Task
A- Action
R- Result
By doing this you will be brief and get your point across in a clear way.

When you go in just be yourself.Always look the person straight in the eyes, sit up tall and proud as if you know that you already have the job, dress for success. You don't want to go into a doctor's office with your breast hanging out.Don't wear alot of jerwerly. That's a big no no.When you shake their hand shake it with a firm grip. Most of all when you first walk in thank them for giving you a second chance.Promise them that you will Never be late, and they don't have to worry about you calling in for work.Last but not least explain to them why you feel you is the best one for the job.It works everytime. Good Luck!

Dress in a black business suit with a white blouse. Look extremely submissive, and don't have fake nails because it is hard to type with them. Have your hair in a tight bun. Walk straight and cross your legs gracefully when you sit. Where glasses even if you don't need them and adjust them often. It worked for me and I now work for Pfizer as the main receptionist.
