Here are some official Fifa rules, that do NOT clarify:

- Up to a maximum of three substitutes may be used in any match played in an official competition organised under the auspices of FIFA, the confederations or the member associations.

- a player who has been replaced takes no further part in the match;
(not clear - can he reenter the match as a substitute though?)

NO in Official games (FIFA and any other Country Football Association).
But in some Leagues (especially youth) it is allowed.

Never seen it happen! Actually, I really wanna know the answer too!! I think they can but they don't do it because they can only do 3 substitutions and they don't want to waste them!

Yes they can, the only time they can't bring someone back into the game is if they got a red card (in which case they can't even be replaced.)
