Coughing increases the intra-abdominal pressure.
The Dr doesn't 'cup' the scrotum while he does this..he either watches for what we call an Expansile Cough Impulse or he palpates the inguinal area to check whether a hernia is direct or indirect.....among other things.....

They're feeling for an increase in air pressure in the area. If one has a hernia (puncture) in the "plumbing" down there, coughing will force air through the hole in the tubing. Er, this is the same mechanism we use to er.... push waste out of the intestinal system. Tis all related.

The doctor is checking to see that the testicles move properly. If they don't, then they have reason to believe that a certain type of hernia has developed.

I don't know...... why did doctors ask a man to cough while cupping his testicles??

I think it prevents tickling or something. I also had a doctor tell me to cough when she gave me a shot, so there must be something to it.
