Well, if you don't need the computer to do calculations (in other words, you just want a paper form) you could use Word, WordPad, or even NotePad to make one up.

If you want something free which is good, try OpenOffice! It's 100% compatible with MS Office, it includes the facility to save as pdf and many other possibilities!

It's free, it's robust, it works, it's like EXCEL. I use both.
