It might. Is the area red and do you feel any swelling or heat at all? If you squeeze the nipple is there any drainage? Seems something triggered this, don't know what you might have done three weeks ago but perhaps some activity instigated this problem. When you say you are experiencing pain, is it constant or just when you move a certain way? The reason for my questions is because most likely a doctor will ask the same things, so you may need to visit a doctor and be prepared to give good details. Good Luck...

As a Doctor I strongly go against piercing of any genital area because of possibility of injury, infection and abnormalities in the future. Tattoos also cause problems if the artist doesn't sterilize the equipment used in the process.
I cannot give a medical opinion since I'm not being paid and I cannot tell what the problem looks like physically. But as a person I suggest you consult your physician.

Maybe. I know your nipple will always be hard though, ugh. See a doctor. Or call the place that pierced you.

The infection could be spreading or something could just be swollen. You should see a doctor.
