I need to see what a subcontract with a provision for living wage looks like?

The subcontract is for plumbing work to be done by someone else, but, billed to my company.

I didn't find any good language in any publicly available forms. Most living wage ordinances require a form to be filed by contractors and subcontractors agreeing to compliance with the ordinance, so any form should reference the ordinance and require the subcontractor to comply with it if compliance by subcontractors is required. What is the ordinance in question?
