could be vacuum, could be fuel issues, could be egr, could be iac....
How does it run normally? What's the year make and model? When did this start? Is it worse in heat, cold, rain, etc?
I need more information before I could even hazzard a guess.

Probably needs a tune up, If you have a clutch it could be the clutch needs adjusted, could need a carburator adjustment, numerous things it could be. good luck.
