Joe Theisman getting his leg broken in two places on Monday Night Football was pretty brutal. I hate the Redskins, but that was not a good thing. I must say though about Theisman as an announcer, I wish someone would break his jaw in two places.
That skier sliding down the edge of the slope or "The Agony of Defeat" guy on ABC's Wide World of Sports, comes pretty close as well.

The worst I saw I cannot say much about, save what I saw. It was a pro football accident. A receiver jumped high in the air to catch the ball, and an opposing player hit him in the legs at the instant he jumped. The receiver flipped over from the impact and landed right on his head. I believe it killed him.

Not sure if it would be classed as worst overall, but the worst that a person has recovered from (in my opinion) was Herman Maier in Nagano 1998 who crashed out of the giant slalom and ploughed headlong into three crash barriers (but managed to win four golds afterwards)

I remember Ray Mancini knocking out game Duk Koo Kim in a hard fought boxing match and Kim died later on from injuries he received. Very sad.

probably not the worst but my favorite. Thanksgiving 2002, 'Skins vs. Cowboys. Brian Barker walking off the field with his nose shoved up into his skull. Great camera work.

From memory his name was Marc Vivienne Foe. A superb soccer/football player who tragically collapsed due to a heart problem on the pitch whilst playing a game. He died later in hospital. So sad.

I don't remember the player's name, because it was a long time ago. But, I saw a hockey goalie get hit in the neck with a slap shot, and it broke his jugular. There was blood pouring out like a faucet everywhere. It was awful.
