"How do I change my name if I am an adult?
If you are an adult, you may petition the circuit court for the city/county in which you live for a determination by the court that good cause exists for the name change. The petition must be made under oath and state the place of residence of the applicant, the names of both parents, including the maiden name of the mother, the date and place of birth of the applicant, the applicant's felony conviction record, if any, whether the applicant is presently incarcerated or a probationer with any court, and if the applicant has previously changed their name, those former names must be listed. Also, the applicant must affirm that the name change is not sought with any fraudulent intent and will not infringe upon the rights of others. A filing fee must be paid at time the petition is filed.

These requirements do not apply to a person seeking a name change pursuant to a marriage or resumption of a maiden name during a divorce proceeding

Both legal parents must agree to the proposed name change. If only one parent files the petition, notice must be given for the other parent to respond at a hearing before the court. The name change does not terminate any legal parental rights"

Depends where you live I think.
In Ohio you go to the courthouse and petition for a name change, then you have to run an ad in the paper for like a month or something and if no-one contests it, Voila, new name. That's what I was told at least.
