Sub-floor, usually 3/4 t&g glued and nailed directly to the floor joists. Then underlayment, this can vary in size from 1/4" to 1/2" depending on whats going on top of it. Then the finish floor. This can be tile, vinyl or hardwood. If hardwood, there will most likely also be a layer of tarpaper between the hardwood and underlayment. If tile, thinset mortar.

Starting from the basement up you have the floor joists, Subfloor which can be 1" boards or plywood, then you have an underlayment as Bear Down stated can range in thickness from 1/4" to 1/2". Sometimes there is NO underlayment and the finish flooring is adhered directly to the subfloor. Finally the uppermost layer of the floor is the finish floor covering be it wood, stone or resilient flooring.

You have wood, as the foundation which are called Joists, those are verticle pieces of wood which support the next layer.
Layer 2 is a layer of thin 1 inch plywood. Next you have your linoleum, Hardwood flooring or tiles.
