Depends on the state, but the state bureau of vital statistics will probably accept a driver's license or similar ID to request a birth certificate. There is probably a written request form, and a fee. I have to get birth/death certificates occasionally, even for other people, and it is fairly easy (if slow) to obtain them. I've had to ID myself, but not with an SS card. Good luck!

Well....you DO exist, otherwise--I wouldn't answer your question, now would I?

Find the address of your area Office of Vital Statistics (normally located either at your State's Capitol building or nearest Metro city). Either over the phone, by mail or even on Internet, give them as much information as possible. The fee shouldn't go over $20 to process this information.

In about "4 to 6 weeks", you should have a shiny new legal birth certificate in the mail.

get a copy of your birth certificate from Tallahassee. You may be able to do it on the web. Check it out. http://www.doh.state.fl.us/planning_eval/vital_statistics/birth_death.htm
