There could be many reasons. You need to look into them rather than just try and solve the problem without diagnosis. Are you particularly stressed out right now? If that's it, try working out before you concentrate on a task. If you are suppressing a problem, stop that, pause and address the problem, or recognize it, articulate it, and say that you are not going to solve it for now, and it's alright. You could have ADHD. If concentration was always difficult, look up ADHD on the web, best to start with the NIH.gov site. Maybe you just hate what you're doing. If you must do it, break the work in segments of time. Finally, if you do not want to diagnoze, yoga is a great concentration help, whatever your issues, including ADHD.

Your body is warning you! Maybe you are too tired, stressed or you are pushing your mind too hard these days or longer. It needs break and......vacation! What I mean by that? Relax a lot, have fun and whenever you feel that your batteries are full, try to concentrate on what you want to do.
If it's more than that, try to drink fresh juices, eat fruit and vegetables, eat some chocolate (chocolate ''stimulates'' your mind and body) and drink a cup of coffee a day (no more than a cup, cos you will have the controverse result).
If there is still no result, you should go to your personal doctor to advice you.

Our mind straying is not allwayas a bad thing. Maybe you're doing too much work? Rest is important too.
But if that's not important then try some caffeine. Keeps me perky for College, and I passed my last maths exam top of class by a whole 9% above the next highest student.
Not too much caffeine, just enough.

Maybe you are undiagnosed ADD like I was. There are medicines like Concerta and Ritalin that could possibly help. Also, try Focus Factor - the commercials are on tv ALL THE TIME. Also, look into herbal fixes like ginseng and that whole family - some people have had success there as well.
