Technically, the king or the queen does not have a surname, but they are head of a particular house. In Elizabeth's case, she is head of the house of Windsor. That's as close to a last name as she gets. It could be worse. The family name used to be Saxe-Gothe-Coulbourg before they changed it.

In view of the anti-German atmosphere of World War I, George V proclaimed in 1917 (on the advice of officials , notably Lord Stamfordham) that all British male descendants of Queen Victoria would adopt the surname of Windsor.

She uses the name Windsor. This has been used by the british royal family since 1917 to conceal their german origins
The tradition in England is for a woman to change her surname to that of her husband when she gets married.
Her husband Prince Philip was born with the surname von Schleswig-Holstein - Sonderburg-Glucksberg, this was changed to Mountbatten before he married (Mountbatten used to be Battenberg, also german).
So if there were any honesty involved in this her name should be Elizabeth von Schleswig-Holstein-Sonderburg-Glucksburg.
