It is made by the testes, ovaries and adrenal glands in adult. In the embryo, it is made by the primitive gonadal streak, IIRC; I may have the name wrong. That sets up a positive feedback loop, that makes the gonads turn into testes instead of ovaries.
Testes make a lot more T than ovaries do, so once they form, you are heading to be a male!
If the adrenals malfunction, making too much T, you end up as "intersex"; your secondary sexual characteristics will be intermediate between male + female.
After menopause, women get by on even less T that we do in our reproductive years, with some nasty effects... :-(
Fat makes some oestrogen; we do not just get oestrogen from our ovaries and adrenals. I am not sure if fat can make T.
The steroid hormones do inter - convert a bit in the body; they are very similar, chemically, and all derive from cholesterol.

When the hypothalamus detects a shortage opf testosterone in blood, it secretes a hormone called GnRh. That causes the anterior pituitary gland to secrete a hormone called LH (Leuthenizing hormone), which, when it reaches the testes, causes the leydig cells to produce and release testosterone. Once the level goes up again, it is detected and GnRH is stopped.
