I was recently promoted. Do I have to specify how much time was spent in each position (I've had 3) at this one employer? Or can I simply give dates of employment and my current title?

The more accurate information , that you provide in your resume, the more likely, that resume will help you in finding, the position that you are seeking. Be as specific as possible, and provide as much positive accurate information as you can. This will make your resume, attractive to potential future employers.

list the total time at the employer and indicate the number of promotions showing increased responsibility you received over that time period

i think you should. this way, you can show your companies that you got promotions and they would be like "wow, this guy's good".
