The worst possible injury(death) was in 1976 the Phillie's left fielder Todd Stamps ran into the bullpen hitting his forehead on a piece of metal piping shattering his eye socket. If ruptured his brain and he later died.

I've seen ankle fracture [Jason Kendall] and lots bean balls [Clemens/Piazza], but the scariest was Kaz Ishii pitching for LA against Houston Sep 8, 2002 catching a line drive flush with his left forehead. A quote from the story: [The batter, Brian] "Hunter drove a 3-2 pitch so hard off the left side of Ishii's head that it ricocheted all the way to the backstop behind home plate for a run-scoring double." The long ricochet in exact opposite direction made it obvious that Ishii had absorbed the full force of the blow.
In fact, it fractured his skull, part of the eye socket. It looked worse than it was, but could have killed him. I'm sure it frightened everyone who saw.

If we are talking about any injury to anybody or anything then it is definitely the poor bird that got hit by Randy Johnson's 100 mph fastball on November 24, 1999. The bird was killed instantly and left nothing except for a few feathers. If you have never seen the video, look it up.

The worst baseball injury I remember was when Jack Hamilton of the California Angels beaned Tony Conigliaro in 1967. This effectively cut short a career that was destined for greatness. Tony C was the youngest player to accumulate 100 homeruns (still hold the record) and was 1970's comeback player of the year, playing for the same team that cut his career short (Angels). Unfortunately he passed away in 1989. I miss him, he was my hero.

Don't know the answer to yours, but the worst I ever witnessed was at a AAA game. A nice outfield, push for the double, hit was made. The batter rounded first and headed for second. Had to slide and went in feet first. Every person in the park heard the leg snap. I can think about that today and still flinch.

Well, the worst injury of all time was during spring training 1974, but it probably wasn't on TV. Carlton Fisk was catching for the Red Sox, and Joe Torre hit a foul tip, which broke his athletic cup, rupturing his testicle.

C Jason Kendall of the Pirate a few years ago hit the bag wrong on a bang-bang play at first, rolled the ankle, and EXPLODED two or three tendons, plus the bone fracture...I can't believe he ever came back from that injury, gruesome!!

See the above--the pitcher who broke his arm during a delivery was Dave Dravecky of the Giants. The player who rolled over and broke his ankle was Moises Alou, then with the Montreal Expos.
