Assuming you want to add a speech balloon into an existing program. You will need to load the picture into a graphics program (such as Paint which is included in Windows - look under All Programs / Accessories). If you want a speech balloon, use the paint brush or the shape tool to draw one. Then simply add text to the balloon where you want it.

You can draw speech bubbles in yourself with a photo editing program like Microsoft Paint, or Photoshop.
