They have maintained separate residences. Do not communicate. One grown daughter. He does still carry here on his insurance because she is a hairdresser and has none but that is about to end.

Separation does not mean divorce. You still have to get the legal paper work done. Check with your state to see how this is done.

No you have to file for a no-fault divorce.

No. They are not divorced until they have a divorce judgment. There is no "common law divorce" anywhere.
