"Transcription" usually means converting spoken text to a format that can be read. There is software that does this (dragon naturally speaking and via voice,) though I have no experience with either of these products. the only reason i mention this is that in the medical industry, "transcript" has a more specific meaning than the general definition i provided at the outset, and we might not be talking about the same business-requirement.
On to your question: computer software is developed by software engineers. There are businesses and individuals like myself and some of the other respondents that will perform this service. Be warned that custom programming is VERY expensive. An off-the-shelf solution like either of the products I mentioned will be much less costly than independently financing this type of project. the original developers of dragon naturally speaking spent years bringing their product to market, and were ultimately forced to sell.

Typically, designing a program of any complexity with style and reliability and real innovation, and planning that project complete with measurable milestones and realistic time-lines and budgets, that gets outside of programming and into the realm of true "software engineering"
That said, there are already several programs on the market that do medical billing. One of those may meet your needs, UNLESS what you're planning is to sell this software to your clients.
Also, there are two basic ways to go here...
1) Software that is installed on the clients machine, and is typically dependent on a specific operating system (unless the application is cross-platform, such as Java or Flash).
2) A distributed application, which can include either a small client application (as in #1) connected to a server running the main program, or a web application where the client who is purchasing the product or service just logs into their member account on a website.

There are thousands of business programs that can possibly be tailored to do what you need. Check with any groups or associations that you deal with. You'll have source it yourself and have the local rep do the training and tailoring.
If you can't find one then do some market research and find out how many others in your line of business around the country would be interested in such a program. Then, write the requirements out as precisely as possible and add flow charts and as much detail as you can. Then get it copyrighted. Then look around the net to find a software developer in your area that you can approach for an estimate on developing it. It may be very costly but someone may actually find it interesting enough to buy it from you and do the developing. Especially if you show that there's a need and a customer base for it.

Well any computer oriented solution is called a software (computer program). For example, for a distributor of goods an "inventory management software" is needed. Therefore, if you want to COMPUTERIZE the system at your place, you need a 'software'. Probably someone who is an independent software developer, who you can hire on a contract basis. You'll have to provide him the specifications for the project you have in mind and he'll come up with an initial design to do the work and how much it'll cost.
Although I do expect there's probably already software out there available for purchase that'll do what you want. Check that out first.

Anyways, if you know someone personally who can code, you can ask them - else if you are okay with doing business online, you can put in your requirements on : http://www.rentacoder.com and let programmers there bid on the job.
You can choose the ones you think are worth and let them make you the product.
