Depends on where it appears. Is probably created by a program running on your PC (graphic programs often use "masks"). So would need to know where it appears (in the root, under your My Documents, under Program Files, etc.) to guess.
But checking for spyware is a good place to start.

Never heard this one before...if it comes and goes it "probably" shouldn't be there...follow the instructions above to be sure you don't have any malicious programs at work!
