Where is a good place to go for dinner for two on about a $150 to $200 budget?
In your answer, it would be great if you could include if you recommend a reservation in advance and if you are aware of the dress code.

I would recommend:
Prime at Bellagio - considered to be one of the best steakhouses in Vegas and it has a view of the Bellagio Fountains out front. Definitely make a reservation and the dress code is "jackets preferred" so get gussied up.
Bartolotta at Wynn Las Vegas - one of the best Italian restaurants in town located right on the lake of the hotel. If you can get a patio seat this would be perfect. Reservations are a must and dress code is the same as above.
Bistro Zinc or Cafe Medici at Lake Las Vegas - the former is American with French influences by Joseph Keller (of Napa Laundry fame), the latter the contintental restaurant at the Ritz-Carlton. Both have beautiful views of Lake Las Vegas. About 30 minutes from The Strip but TOTALLY worth it. Reservations recommended and dress code is upscale casual (don't need a jacket).
BOA Steakhouse at Caesars Palace Forum Shops - Try to get one of the tables out on the patio overlooking The Strip. Amazing food, terrific service. Reservations recommended, dress is upscale casual.
Alize at the Palms - perfect French cuisine from local legend Andre Rochet with a view from the top of the Palms tower that is hard to beat. Reservations required, jackets are not.
Those, to me, are the best combination of atmosphere and food and although a couple will push your $200 limit you should able to get out of them for 2 people at that kind of cost.
Other great atmosphere options inculde Hugo's Cellar at Four Queens downtown (every lady gets a rose), Top of the World at Stratosphere (rotating restaurant 100 stories up), the Eiffel Tower Restaurant at Paris (great views), and Le Cirque at Bellagio (French). The problem is that the food is not quite up to the atmosphere and not worth the prices most of them charge, in my opinion.

For my parents' 50th Anniversary last year, I arranged for a "Tasting Menu" at Andre's. It included Soup, Salad, an appetizer, main course, vegetable, starch, dessert, and coffee or tea. It was, I believe $75 per person plus tax and tip. My parents said it was the most romantic dinner they ever had.

That's where my husband and I go for romantic dinners in Vegas.
The food is fabulous and the wait staff is superb.
We went a few weeks ago and the final bill was exactly $200 with drinks, dessert and tip.
The dress code is business casual. No coat and tie required. But you probably wouldn't want to go in a shorts and a t-shirt either.

"The Steakhouse" at Circus Circus is considered by many to be the best steakhouse in Vegas. Not really romantic though.
Any restaurant at Bellagio or Mandalay Bay is excellent and elegant.

dont know much about vegas, but if youre ever in galveston tx there is a fabulous place called PLAYA De LORO on the west beach, self serve salsa bar, killer margaritas, great service and its very casual, shorts and swimsuits but still the ambiance makes it feel like a 5 star place, shrimp and goat cheese enchiladas, chile rellenos, tilapia tacos, hope to see you there if ever in galveston
