You may have a dog that is coming and peeing in your yard, the acid from the urine will make those areas die.

First you can rake out dead patches and plant with rye. Once the rye starts to grow then you can add the seed for the type of grass you have. Also a good fertilizer and enough water to keep the new seed and grass moist. Hope this help.

could be grubs or moles..get treatment and aerator from local hardware..or it could it be a water or shade issue with your yard.

sounds like it could be grubs... Scotts makes a treatment for them. Try it

You might have a fungus. I heard sprinkling a little Tide washing powder and watering it will kill it. (And Tide is biodegradable)
