According to the Baseball Hall of Fame and assuming you meant any game in general, rather than only MLB, the longest game was 33 innings.

Some interesting tidbits from the longest game in professional baseball history - a 33-inning marathon between the Rochester Red Wings and Pawtucket Red Sox that began on April 18, 1981 and concluded nearly two months later:

Extra special: The game surpassed by four innings the previous longevity record, established by Miami and St. Petersburg of the Florida State League in 1966.

Time of game: 8 hours, 25 minutes. Achy, breaky knees: Wings catcher Dave Huppert caught 31 innings.
