If you're old enough to go to the gynecologist, you're old enough to say "Mom, I love you, but I really don't want to lay on a table naked with my feet up in stirrups and my legs apart with you in the room. I'll be right back."
P.S. If you're not old enough to drive yourself to the gynecologist, you're probably not old enough to have sex.

You can ask the Dr. or his nurse privately to not have your mom in the room while you are talking with them. We (Dr's office) usually told the parent that is was much more comfortable for both the dr and the patient if they could go through the exam without an audience. Usually the Dr. would have the discussion with the patient during this time.
As a mother of 4 (1 girl) I really didn't feel it was my place to be in there as that was what I considered an invasion of privacy to my daughter but I must add that we had very open discussions, so she and I had no reason not to be honest. Her sex life was her choice but I wanted to make sure she had every bit of information to make responsible decisions and she had so far !

Politely ask her to wait in the waiting room, that you're getting older and think that you should go into the exam room alone.
And depending on your age, it may be required by law for her to remain in the room (even with a nurse present) or be involved in the questioning/treatment. You can always call the doctor's office and ask what their policy is, that way you can be prepared.

If you're not old enough to go by yourself and you don't want your mom to know why you're going, just tell her you think it's about time you start getting regular checkups. You can say you saw the new ads on TV about cervical cancer and want to get a pap smear. Tell her you want to go in alone because you're going to be very exposed and you're not comfortable with anyone but the doc (who HAS to look for the checkup) seeing all of you. She should understand that. Once you're in the doctor's office, explain to the nurse or doctor what's going on.

When i would go for physicals at my doctors office they would ask the personal questions while i was in another room with the nurse, but i would just tell your mom that having her in there makes you feel uncomfortable.

oh man from my experience the doctor asked me in front of my mom if i wanted to answer questions in private and i had to say no cause you know if you say yes your mother is gonna flip cause you're keeping "secrets" from her...if i were you i'd try to get the message to the doctor that you want to see them in private for example pass a small note to the receptionist etc...if i have any other ideas i'll let you kno...
(ps i dont go with the if youre not ready to tell mom then youre not ready for sex cause i'm a woman but i know my mother wanted me to wait for marriage and i just dont want to hurt her so deeply...but i hope you are old enough...)

She can take you and you can tell her that you're going in alone because you don't want her to see your "private parts" and you tell your doctor when you're in the room with him that he has to keep things to himself "Doctor/ Patient relationship" he can't give out information unless you want him too and unless it's harmful to your or anybody elses health. So don't worry. You can also ask a friends mom to take you. If you're 18 of course.

Tell her that it is a private thing and you don't want anyone but the doctor looking at you. Tell her you are a big girl and that you don't need her back in the examining room

Ask her to stay in the waiting room. Let her know it makes you feel uncomfortable. If she has any questions to ask the doctor, she can do so later. When you're clothed and standing upright.
