Collect income taxes on the spot from all illegal immigrants who come in for services. Anyone who says they don't have money, not a problem. Make them give two pints of whole blood. Unable to give blood? Not a problem. Make them do maintenance work on the facilities, like painting, plumbing, cleaning, etc according to their skill level. Unable to do work? No problem. Make them do eight hours of translating service. Not bilingual (can't speak English)? No problem. Just tell them you don't speak whatever language they speak, and after a few hours of not getting anything they came for, hopefully they'll leave and not come back.
LOL. Sorry if I put anyone off with this reply, but you were looking for humorous replies, right? I mean, you did ask how to make the NHS more efficient and cost effective. Now, that's a joke!!!

'ladyburlesque' almost summed it up for me, except for the 'charity' bit! NHS should never be regarded as any form of charity, far too many people already think of it as a charity!! It is not and it was never designed to be. The NHS is (or should be) fully funded by insurance payments from young fit and healthy workers, we all know the history!!
The problems with any business going bust are almost always down to bad management , and the management of the NHS is not just down to local hospital management, it's down to the Government.
Can it be really that difficult to work out that we should need X amount to run it, and we have Y amount coming in to pay for it, but above all we need a Government that knows how to handle it .
The Government we have now is no better or worse than any of the other political parties, all full of wind and pi**!
Today's workers are paying for these excessive Government actions of 'throwing money' at the problem already, the blame lies at the top.
Having got that off my chest the question remains how to solve the problem? The Government are clearly incapable of working out an efficient programme, they must employ some one that can, and if it is decided at the end of the day that outgoings exceed incomings the insurance has to go up!! Could it be as simple as that?

more training for hospital staff, better volunteering service for those who are interested in careers in the medical world. Charity and fung raising events.
Educating the hospital managers in economics, budgeting and priortising. More doctors and nurses employed as to many hospitals are understaffed.

Answer to this is simple, scrap it!! We should adopt the system the americans have of private hospitals and health insurance, this could be paid for from the tax we could save as individuals if we weren't funding this out dated system.
