Please kindly sign your name on the new/renewal lease as you agree with the terms and conditions which listed here. If there is any disagreement, please mark and give signature beside the disagreement of the terms and conditions. Furthermore, please give a brief signature at the comer of every single page of the leases. Thank you.

Please sign and return at your earliest convenience. You may change the courses if necessary, provided, you have to put initial on each alternation & sign every individual page on the conner of right hand side.
