他當初並沒有帶她穿越那條河, 所以出事了.

假設語氣要表達"與過去的事實相反"的語意, 用過去完成式:

If he had taken Lili to cross the river, nothing would have happened.

If he took Lili to aross the river, everything would be not happen。
=>If he took Lili across the river, nothing would have happened.

=>If he had took Lili across the river, nothing would happen.

應該是以下這樣

If he took Lili to aross the river, everything would not be happen。
