"Um homem grande" we use to call the guy is tall, or fat. "Um grande homem" is kinda how we say "He is a great guy, a wonderful person".

Usualmente se considera um homem grande como sendo um homem de grande estatura. Um grande homem é um homem de grande importância. Mas na verdade essas duas frases podem significar a mesma coisa.

Um homem grande tem alta estatura física (a big man).
Um grande homem tem grande estatura moral (a great man).

In portuguese the 'default' position for adjectives is after the noun. In most cases, you may put the adjective before the noun, without any change in meaning. (though the sentence would sound "marked," bordering the ungrammatical if there be no justification in style.)
However, there are a few idiomatic usages. Thus, "um homem grande" means "a big man" but "um grande homem" means "a great man." (i.e. good, honourable, etc)

Um homem grande = Um homem ALTO ( Estatura alta)
Um grande homem = Alguém que fez algo de muito bom, seja ao próximo, ou à sociedade.
