Algumas que o pessoal colocou acima não é que estejam erradas, mas fogem ao do inglês praticado normalmente nos EUA.

Muitas das respostas dadas até agora estão erradas na tradução da segunda frase, onde o verbo está no tempo imperfeito:
"How old are you?"
"Which city in the USA did you used to live in ?" ou "Which city in the USA did you live in" (ou "In which city in the USA did you used to live" - frase gramáticamente correita mas um pouco pedante)
"How was (your) school in the USA?" ou "What was (your) school like in the USA" (A palavra "your" pode ser omitida nas duas formas da frase).

todas as respostas dadas até agora estão incorretas...até dá pra entender mas as traduções corretas mesmo são:
**How old are you? = quantos anos você tem?
na segunda frase: em que = "which in" // a preposição "in" vai pro final da frase.
o verbo "morar" está no pretérito imperfeito então usa-se
" used to live" = morava
mais um exemplo: I used to speak = eu falava
Então:
**Which city of the United States did you used to live in?
Em que cidade dos EUA você morava?
** How was the school in the United States?
como era a escola nos EUA?
