That depends. Some people perfer the pronounciations first, but others may take precedence to certain kanji. There is no absolute, it’s a two way street out there.

I don’t know how many people out there have mixed (kanji + kana) GIVEN names. These forenames do exist in Japanese name databases, but I guess these are not real names, just stage names or whatever. Also, 平仮名 and 片仮名 do NOT coexist in a given name.

I found a bunch of mixed kanji + kana forenames from here, you can go check it out. You will however need to punch in the pronounciation before accessing to the database:

Addendum for (2):神田沙也加 IS the real name. Seehttp://ja.wikipedia.org/wiki/神田沙也加Sorry but I might not be able to help you with (1). There are, however, baby-naming books that teach you JUST that.
